In fast ticker finished 2nd to Real Madrid in the group stage, now playing them in the next round.

There was never a rule in the game preventing that, the only rule is to avoid same country matches whenever possible. We have that in our list to fix but with a very low priority :-)

FFS, again exactly the same issue, a season or two later, this time I won the group and they came second and we play each other in the next round, any chance of fixing this?

No immediate plans to fix that as the focus of the pairing logic is prevent teams from the same country facing each other. But it will be done eventually, just not a high priority.
